| 3. Problem/s I faced: | I was asked to pay money for the form to be accepted by the person to whom the form was supposed to be submitted |

| 5 a. Was the bribe paid? | I had to pay for two reasons first was that the person with whom form was to be submitted itself told that it wont be possible without bribe i may not have paid but it was the last date |

| Suggestion 1: | clearly mention using pemplate in office itself that do not pay bribe. i have already seen such pemplate in banks |
