| 1. Period of incident: | 2009 – Mar |
| 2. Services that were sought: | Arrears and Travelling Allounces These should have been transfered in to the respective accounts of the employees.But deliberately the Medical officer Incharge, primary health centre, Rohta, District Meerut ,withdrew the amount from the bank and distributed in cash. |
| 3. Problem/s I faced: | The Medical Officer Incharge and Accountant asked for 30 to 50 percent of the sum as bribe.As I opposed to give such a large amount of bribe (i.e. about Rs.2000), they intimated me of dire consequences |
| 4. Bribe amount: | Rupees 2000. there are about 50 employees at the centre. Now amount of the bribe can be calculated |
| 5 a. Was the bribe paid? | Yes, I had no other alternate.I was intimadated for dire consequences |
| 5 b. Could the work have been done without paying the bribe? If so, how? | |
| 6. Result: | After taking the bribe they gave me the rest of the amount. |

| 1. For head of government department: | |
| Suggestion 1: | I request to The Honourable Director General Of Medical Health Directorate Hajrat Ganj Near High Court Lucknow,Uttar Pradesh to stop such incidents and give directions to all the concerned authorities not to make any payment in cash.Such types of incidents affect our sincerity towards our duties. |
| 2. For the Anti Corruption Bureau (ACB) / Central Bureau of Investigation (CBI): | |
| Suggestion 1: | If any corrupt official is given punishment then the other corrupt offials learn a lesson from it. The medical Officer Incharge Of The Primary Health Centre,Rohta has been withdrawing the arrears, bonus and travelling allounces in cash for several years. Thorough investigation should be done because there is documentary proof of the incidents |

| Suggestion 1: | Every person should be aware of his rights.He should inform to the concerned departments (i.e. Higher Authority and anti corruption departments. |
