| 3. Problem/s I faced: | Cardiogram was not given to the Patient.Patient subsequently died of Heart Attack |
| 4. Bribe amount: | Rs. 1000/- |
| 5 a. Was the bribe paid? | Rs. 1000/- was collected from the deceased patient for the Cardiogram as well as a Donation for the Cardiogram Machine |
| 5 b. Could the work have been done without paying the bribe? If so, how? | The Cardiogram could only be purchased out of Donations collected from Patients |
| 6. Result: | After the patient died,a Doctor from the Medical Centre revealed that the Cardiogram showed an Electrical Defect in the Patient’s Heart ,which could have been treated ,had the Cardiogram been given |
