| 1. Period of incident: | 2009 – Mar |
| 2. Services that were sought: | I am employed in Health and Family Welfare Deptt, Meerut, Uttar Pradesh and my arrears was pending and I did not get it until 15- April |
| 3. Problem/s I faced: | Accordinfg to the rule my arrears should be transferred to my bank account but the medical officer incharge and account enchased my arrears from the bank and asked me to pay 30% of the total amount. |

| 5 a. Was the bribe paid? | Yes I have no any other option (They threaten me to teach a lession if I refuse to pay the bribe) |

| 1. For head of government department: | |
| Suggestion 1: | Thorough enquiry should be made against the Medical office incharge and accountant. In future no payment should be distributed in cash. They used to encash arrears, bonus, travelling allowence other benifits and asked bribe to hand over us. |

| Suggestion 1: | issue directions to principal secretary (Health and Family Welfare) and Director General – Health to catch the corrupt officials and do the needful to prevent these type of activites |
