B: My Complaint Details
1. Period of incident:	2010 – Apr

2. Services that were sought:	change address for refund cheque

3. Problem/s I faced:	assessing officer asked for bribe and thretned for cancellation/penalties on PAN if I refuse to pay bribe.

4. Bribe amount:	10% of refund amount

5 a. Was the bribe paid?	no

5 b. Could the work have been done without paying the bribe? If so, how?	not yet

6. Result:	still waiting for address change for my refund cheque

D: My Suggestions
1. For head of government department:
Suggestion 1:	corrupt govt employees should be immediately terminated.

2. For the Anti Corruption Bureau (ACB) / Central Bureau of Investigation (CBI):
Suggestion 1:	Please take some serious action, as in that department this problem is quite frequent

3. For other citizens:
Suggestion 1:	never pay any bribe for any govt work
